Right of Way Permit Application (Excavation & Obstruction)

This permit application is for working in, opening, or placing an object in a city street or public property within the city limits of Fort Dodge, Iowa. 

A Right-of-Way (ROW) Permit is required for any work being performed outside the private property, which may include but not limited to the following:

  - Driveway Approach or Curb Cut Repair/Installation   - Dumpsters
  - Public Sidewalk Repair/Installation   - Power Lines
  - Utility Repair/Installation (water, gas, electrical, fiber, etc)   - Trees
  - Telecommunications Repair/Installation   - Mailboxes
  - Sanitary or Storm Sewer Repair/Installation (addtl permit req)   - Small Wireless Facilities (addtl permit req)
  - Water Service Repair/Installation (addtl permit req)

Permit applications, once submitted, will be reviewed for approval. All required information must be supplied. Once data is available, the review process will evaluate the location, type, and date of the work, as well as the traffic control plan and status of the contractor's registration. All circumstances will be considered, especially the impact on the public and public safety.

If additional information is required, contact will be made by email or phone to process the permit. Plans and additional documents can be emailed to engineering@fortdodgeiowa.org

Once approved, an email will be sent from the system with an approval statement. Construction may not commence until approval is received. A copy of the approved permit, must be displayed at the site. 

If information submitted on the application changes, the contractor or homeowner, whoever is responsible for the work, must contact the Engineering Department. Any change may cause delay in processing if additional approval is needed due to public circumstances. 

Upon submittal of this application, you shall be under the compliance of the City Code of Fort Dodge and the following paragraph:

In accordance with the provisions for Section 573 of the Code of Iowa, I, the undersigned, agree to warranty the work set out on this permit, from the date of completion and inspection, for a period of not less than 4 years for Storm/Sanitary Sewer, Water Main, and Streets, and 2 years for PCC Patch work. The undersigned guarantees to perform this work in accordance with the plans and specifications approved by the City Engineer, and in compliance with the Ordinances of the City of Fort Dodge, Iowa.

NOTE:

The Applicant is the person or contractor registered to work in the ROW.

The Registrant is the owner of an obstruction to be placed in the ROW.

The following documents must be submitted before this permit can be approved: - $10,000 Bond on the City's form - Construction Plans - Traffic Control Plans - Payment - Other registration requirements (application, fee, insurance, etc) All documents can be emailed to engineering@fortdodgeiowa.org.
