City Employee FFCRA Emergency Leave Request Form 

In a position where you need to use the leave available to you under FFCRA? Please complete and return the above form to Human Resources. We must have this form to be able to classify your time accordingly.

City Employee Telework Agreement 

The Telework Agreement must be completed for all employees who have been authorized to work from home during the COVID-19 pandemic and shall be maintained in their personnel file.
